Treatment of Periapical and Pulpal Pathologic Involvement in Premolars Due to Dens Evaginatus

Abstract
A 21-year-old Korean woman presented with mucosal swelling and sinus opening at the apical area of the maxillary left/right second premolar, mucosal swelling and intermittent dull pain at the apical area of the mandibular left first premolar and severe pain of mandibular right first premolar Periapical radiolucencies were. observed with the maxillary left/right second premolar and mandibular left first premnlar. No change except the loss of lamina dura and the thickening of periodontal membrane on the periapical film of the mandibular right first premolar was obse¡©rved. Clinical examination revealed attrited protuberances on the occlusal surfaces of the premolars.
A diagnosis of dens-evaginatus with associated periapical lesion secondary to pulpal necrosis was made.
Conventional root canal therapy was performed with the corresponding tooth and finally the canals were sealed permanently with gutta-percha and zinc oxide-eugenol sealer. Six months later, standard periapical films of the corresponding teeth showed the increasing periapical radiopacities suggesting bone regeneration. All the premolars fuctioned normally.
